Our mission
Bridging the gap: a dental care model for Medicaid and Medicare
FulcrumCare is building a new dental care model for Medicaid and Medicare beneficiaries. Our patient-centered model includes community-based services, care coordination, and collaboration with primary care teams to ensure access to dental care. By leveraging interoperability and population health technology, we enable seamless data sharing and care coordination across teams to better serve Medicaid and Medicaid patients.
By addressing unmet dental needs, we improve whole-person health outcomes, support our clinical partners in achieving their population health care goals, and reduce avoidable downstream healthcare costs.
